Facility Requirements:• Must have BLS certification (Must be through the American Heart Association)• Must have mininum 4 years recent RN Case Management or Licensed Clinical Social Worker experience, with experience in a LTAC or STACH setting, MCG, discharge planning across acute care settings, strong IDT collaboration• Must have New Jersey RN or multi-state RN or NJ SW license• Shift is Monday - Friday 8a-430p
