Job Description

            Clinical Case Manager     

REPORTS TO: Life & Resiliency Skills Director

GENERAL FUNCTION: 

The Clinical Case Manager (CCM) is responsible for providing high-level case management services to Boys & Girls Club members and their families and supervising all Medicaid billing-related case management services provided by Boys & Girls Clubs of Broward County (BGCBC). This position ensures that all case management services are delivered in a trauma-responsive, strengths-based, and culturally competent manner. The CCM oversees the case management-related job segments of the Life & Resiliency Skills Coordinators including crisis intervention, psychosocial assessment, family intervention, member/family education, and service planning. Additionally, the CCM is responsible for supporting BGCBC’s mission through the practice and implementation of ethical decision-making, Medicaid compliance, clinical documentation, and partnerships that enhance the mental health and social-emotional well-being of youth and families served through BGCBC.

KEY ROLES:

Clinical Leadership & Supervision

Provide clinical supervision and mentorship to Life & Resiliency Skills Coordinators related to all aspects of case management and Medicaid billing including but not limited to: Providing one-on-one and group supervision on an as needed basis, 
Providing crisis intervention support as needed, 
Oversight of the contractual agreements and ensuring staff are providing services agreed upon to Boys & Girls Clubs of Broward County’s standards,
Reviewing clinical documentation and ensuring that it is completed and submitted according to established time frames, and 
Providing oversight to maintenance of the client files to maintain compliance with contractual agreements.
Ensure that the Life & Resiliency Skills Coordinators adhere to ethical and professional standards of clinical practice and documentation.
 

Compliance & Documentation Systems

Establish, implement, and maintain a charting and clinical documentation system compliant with Medicaid and agency requirements.
Assure that all clinical documentation is completed according to BGCBC policy and procedures, contract requirements, and all other applicable regulations.
Oversee the billing of claims submitted under BGCBC’s name, serving as both the supervising and rendering provider.
 

Assure all required billing and data paperwork is completed accurately and within required timelines. 
Maintain confidentiality and compliance with HIPAA and other applicable regulations.
 

Program Development & Collaboration

Develop and maintain quality relationships with schools, community organizations, and mental health providers to create a continuum of care that supports the holistic needs of youth and families.
Develop and oversee a Social Work Internship Program in partnership with local colleges and universities.
Support the Life & Resiliency Skills Director in the ongoing development and implementation of trauma-responsive, resilience-building, and SEL initiatives across Club sites.
 

Administrative & Operational Responsibilities

Provide services to Club members and their families with meet current and future psychosocial needs.Responsible for all aspects of the case management service planning process including but not limited to the service plan and subsequent reviews, safety planning, discharge planning and crisis stabilization planning. 
Ensure that all case management services are delivered in a trauma-responsive, strengths-based, and culturally competent manner to ensure alignment with policies and procedures, best practices, and BGCBC’s mission.
Report issues and concerns regarding case management services or billing to the Life & Resiliency Skills Director in a timely manner. 
Participate in assigned training and school or community meetings as required for the work of our Life & Resiliency Skills department.
Assures the accurate collection and reporting of outcome and programmatic data to internal and external sources.
Work outside of designated hours as needed to respond to crises involving Club members, families, or staff.
Oversight of the contractual agreements.

ENVIRONMENTAL AND WORKING CONDITIONS:

Most of the work will be conducted in an office setting within the assigned Club(s); however, some blocks of time may be spent away from the office, providing direct services to the youth and staff in the Clubs.  Various off-site meetings will occur at respective Boys & Girls Clubs. 

PHYSICAL AND MENTAL REQUIREMENTS:

High energy level required. Must be comfortable performing multi-faceted projects in conjunction with day-to-day activities; must possess superior interpersonal abilities; and the ability to always get along with diverse personalities displaying tact, maturity, flexibility and professionalism. Good reasoning abilities and sound judgment are also required. Successfully communicate through speaking, writing and reading with comprehension. Manage multiple tasks with varying deadlines.

Position requirements include:  

Walking–during events and other activities, required to walk (for up to 4 hours) throughout as needed
Talking – constantly required to express or exchange ideas by means of the spoken word 
Hearing – frequently required to work in a loud environment (primarily events)
Sight – position requires both close and distance visual acuity for computer work and monitoring volunteers
Standing – occasionally required to stand for sustained periods of time (related to Club activities and Organizational events.) 
Sitting – frequently required to sit for sustained periods of time (related to case management and administrative activities.)
Bending – occasionally required to bend (picking items up off of the floor, assisting in set-up and break down of events)
Dexterity – occasionally required to type for extended periods of time (answering emails, writing reports, creating program documents, etc.) 
Lifting – occasionally required to lift/move items up to 25 pounds.
Skills essential for successful communication include speaking and writing. 
Workers will be subject to both inside and outside environmental conditions.

Requirements

QUALIFICATIONS/REQUIREMENTS:

Must hold a master’s degree in social work or a related field and maintain licensure in the state of Florida as a Licensed Clinical Social Worker (LCSW.)
Licensed as a LCSW Supervisor in the State of Florida and have a minimum of one year of supervisory experience.
A minimum of one year’s experience with Medicaid billing.
Certified Behavioral Health Case Manager (CBHCM)
A minimum of one year of experience providing services to youth, adolescents, and families in diverse communities.
Four (4) years of clinical social work experience and sufficient post-licensure experience to qualify.
A graduate-level academic course in supervision that meets the requirements of Rule 64B4-6.0025 (Fla. Admin. Code) 
A continuing education course in supervisory training that meets Rule 64B4-6.0025 
A post-graduate training course for field instructors in clinical social work 
Or designation as an approved supervisor by a professional organization (e.g. AAMFT, where relevant) 
Ability to utilize independent judgment and take initiative within BGCBC policies and procedures as well as the scope of the position.
A safe driving record is required and must be maintained.
